Earlier this summer, Fallout 3 was refused a rating by the OFLC(the Australian equivalent of the ESRB) due to the realistic portrayal and use of drugs in the game. This would have lead the game to be banned from shelves in Australia unless the game was altered and then resubmitted for classification. Bethesda altered the drugs and cut the animation showing their actual use, and the game was Australian version was reclassified as MA15+.

Bethesda’s upcoming title Fallout 3 has been refused classification by the Australian OFLC. Any Video game in Australian which is given an RC rating cannot be sold or distributed in the country legally.
This shouldn't come as a surprise given the strict ratings systems currently in place in Australia....

Bethesda have revealed that the Fallout 3 Survival Edition will not make its way to Europe. The Survival Edition contains a life-size replica of the wrist-mounted Pip-Boy 3000 worn by characters in-game, a Vault-Tech lunch box, a Vault Boy Bobblehead, an Art of Fallout 3 book and a making of DVD and will be released later in the year.
